Community Health Worker

Job Summary
Support staff and families in the Functional Family Therapy – Gang program by practicing skills with clients, assisting with administrative duties, and managing community outreach. Upload assessments and contact notes to Electronic Health Record systems, collaborate with clinicians to support youth engagement, and act as a mentor for relationship building. Attend intake and sessions as needed, participate in community events, and provide recovery support while maintaining valid driver's license and auto insurance for state-wide travel. Requires high school diploma, one year of relevant experience, and bilingual Spanish skills preferred.
Required Qualifications
- High school diploma or equivalent
- CHW certification
- Minimum 1 year's work or lived experience related to the position
- State wide travel
- Strong organizational, critical thinking and time-management skills
- Basic computer skills
- Knowledge of Microsoft Office 365
- Ability to communicate and effectively interact with a diverse population
- Must authorize Child & Family to complete a motor vehicle records check
- Valid driver's license
- Auto insurance
- Car registration
- Ability to meet all background checks and medical clearances required for the position
- Ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, and races
- Demonstrate knowledge of the history of discrimination in America, particularly as it relates to race and racism, and how this history has led to disparities experienced by marginalized communities such as BIPOC, people with disabilities, LGBTQ+ communities, and others
- Ability to successfully deliver culturally responsive services
