Job Summary
Develop and implement tailored care plans and SMART goals for high-utilizer populations to reduce emergency department reliance and improve health outcomes. Administer the PRAPARE screening tool to identify social determinants of health needs and connect patients to community resources, referrals, and program incentives. Ensure continuity of care by monitoring recently discharged patients and proactively scheduling follow-up appointments within seven days of hospital release. Partner with healthcare providers, data analysts, and community agencies to manage cohort lists and present updates at interdisciplinary team meetings. Document encounters and vital clinical statistics in the Electronic medical Record within 24 to 72 hours and maintain SharePoint databases for grant reporting.
Required Qualifications
- Valid driver's license
- Reliable personal transportation
- Valid Basic Cardiac Life Support certification
- Occupational Safety and Health Administration safety training
- TB skin test or chest x-ray
- Hep B immunization
Desired Qualifications
- Completed Medical Assistant, Community Health Worker, or Health Educator program
- Three years of clinical work in an outpatient setting
