Community Health Worker

Job Summary
Conduct outreach between 12-8:30pm across the 16 parishes VOASELA cover to verify homelessness and connect Veterans and community members with rapid re-housing and homeless prevention services. Perform strength-based needs assessments, refer individuals to case management teams and intake specialists, and provide direct assistance including transportation, communication, and self-advocacy support. Engage with landlords, homeless shelters, local VA offices, GPD sites, and the Community Resource Referral Center while advocating for services and resolving conflicts. Attend scheduled Stand-Down events and community gatherings. Maintain compliance with training via the Relias Learning Management System and adhere to designated dress code.
Required Qualifications
- High school diploma
- Current driver's license issued by state of residence
- Current Vehicle Inspection decal
- Current vehicle insurance
- Current vehicle registration in state of residence
- Satisfactory Motor Vehicle Report
- Must be able to pass a criminal history investigation
- Must self-identify as have a disability and receive or currently receiving services
- Must be able to lift up to 25 pounds
Desired Qualifications
