Job Summary
Conduct outreach for health initiatives advising individuals on diabetes, obesity, prevention, and immunization while identifying high-risk groups for one-on-one coaching. Act as a resource to clients and providers by distributing educational materials, attending community meetings, and advocating for health needs with government agencies. Provide cultural mediation and remove barriers to care for underserved populations including minorities, low-income residents, and pregnant women. Manage work independently with occasional weekend coverage.
Required Qualifications
- High School Diploma/GED
- Experience working in the community
- Experience with Community Organizations and Home Health follow-up
- Ability to work independently, set priorities and manage work within timeframes, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude
- Strong customer service orientation and the ability to handle highly confidential information essential
- Must be computer literate
- Will complete Community Health Worker (CHW) or Promotoras training
- Occasional Weekends
