Job Summary
Conduct culturally responsive home-visiting, education, outreach, and care coordination for women, pregnant and postpartum individuals, infants, and families. Build trusted relationships by providing peer support, resource navigation, and assistance with transportation, interpretation, and telehealth access. Collaborate with social workers, nurses, and healthcare providers to participate in multidisciplinary care planning and support patient enrollment and engagement. Maintain an active caseload by documenting visits, referrals, and services accurately while following organizational policies and confidentiality requirements. This full-time role (40 hours/week) typically works Monday through Friday within the Strong Beginnings program at the Westside Health Center, with occasional evening or weekend hours as needed. Bilingual proficiency in English and Spanish is required, with Mam or K'iche' also acceptable.
