Job Summary
Community Health Worker for Community Connect who engages individuals and families in underserved areas, conducts in-person visits, and facilitates access to medical, behavioral, and social services. Builds trust, conducts health education on chronic disease management, preventive care, nutrition, mental health, and wellness; connects clients to housing, food access, transportation, and behavioral health services; assists with health benefit exchange navigation; advocates to reduce barriers; documents interactions per HIPAA; collaborates with clinical and social service partners; participates in team meetings and continuous quality improvement; serves as a cultural broker to ensure culturally and linguistically appropriate services; travel to client homes and community locations as needed; expects regular travel within designated service areas and adherence to privacy standards.
Required Qualifications
- High school diploma or GED required
- Completion of a certified Community Health Worker training program (or willingness to complete upon hire)
- Completion of health benefit exchange navigation training (or willingness to complete upon hire)
- First Aid/CPR certificate (not required, but preferrable)
- Valid driver's license in state of residence, or ability to obtain prior to employment
- Must have reliable transportation
- Must maintain automobile insurance as required by the State of Washington
- To support operational needs and compliance requirements, this position is expected to be performed within the State of Washington.
