Community Health Worker

Educate patients on community resources, help them set health goals, and assist with transportation and appointment access. Navigate patients through the care continuum, including scheduling, visit completion, follow-up coordination, and physician verification. Provide health-related lectures and offer language translations at the hospital and in the community. Collaborate on program strategy, report barriers and successes to the coordinator, and help members find services like rent relief, housing, and food assistance. Complete Spanish interpreter certification and direct applicants to Medicaid resources.
