Job Summary
Community Health Worker role within SIHF Healthcare, serving clients in both health center and community settings. Responsibilities include managing a caseload, screening for health-related social needs with the PRAPARE tool, initiating referrals, contacting clients to discuss and schedule health screenings and follow-ups, assisting with health information and paperwork, coordinating with healthcare and social service providers, participating in community events, and maintaining accurate records in the CHW database. Requires strong interpersonal skills, a desire to help others, knowledge of local community resources, ability to work in office and community environments, and basic computer/technology proficiency. Education requirement includes a high school diploma or GED, with a valid driver’s license and reliable transportation; lived community experience is valued. Full-time schedule with local travel and occasional out-of-town travel; benefits package available for full-time employees.
