Job Summary
Hybrid Community Health Worker / Care Coordinator in Nevada, supporting screening, assessment, and care navigation for the Path Assist program. Role involves telephonic outreach, SDOH screening, intake and assessments in the Activate Care platform, creating client-centered care plans, coordinating with local nonprofits and resources to meet client needs, and providing resources to improve health literacy and self-sufficiency. Must be able to commute to and from clients’ homes, reside within 30 minutes of Carlin, NV, and work hours 9:30 AM–6:30 PM, Monday–Friday. Responsibilities include care coordination for an assigned caseload, enrollment assistance for services/benefits/programs, maintaining client privacy, and participating in weekly team activities. Requires a high school diploma or equivalent, Nevada driver’s license, personal vehicle for commuting, and 2–3 years of relevant experience in care coordination or related settings. The company notes no visa sponsorship currently and supports a hybrid work arrangement.
Required Qualifications
- Degree requirements: minimum high school diploma or equivalent
- Must have a valid driver's license in the state of Nevada
- Must be able to use personal vehicle to commute to and from client’s homes
- 2-3 years of relevant work experience providing direct care coordination services to individuals and families (preferred)
- Experience working with nonprofits, social service providers, faith-based groups, or government agencies addressing social determinants of health
- Independent working skills with strong communication
- Collaborative team player
- Cultural humility and ability to work with diverse populations
- Additional language skills are a plus
