Job Summary
Hybrid Community Health Worker role in Winnemucca, NV focused on care coordination and resource navigation for local Nevada residents enrolled in the Path Assist program. Responsibilities include telephonic outreach, enrollment coaching, administering social determinants of health (SDOH) screenings, creating client-centered care plans, coordinating with nonprofits and local resources, and documenting client interactions in the Activate Care platform. Required Qualifications
- High school diploma or equivalent
- Valid driver's license in the state of Nevada
- Must be able to use personal vehicle to commute to and from client’s homes
- 2-3 years of relevant work experience providing direct care coordination services to individuals and families (preferred)
- Experience with nonprofits/social service providers/government agencies addressing social determinants of health
- Strong independent work skills and effective communication
- Collaborative team player
- Cultural humility; ability to work with diverse populations
