Job Summary
Connect patients to healthcare and community resources while helping individuals overcome barriers to care. Support chronic disease management and healthy lifestyles by providing health education and advocacy. Work alongside providers, nurses, and care coordinators to deliver comprehensive services. Participate in community outreach activities to expand access. Monday–Friday 8-hour days with no nights, weekends, or holidays. Bilingual (English/Spanish) preferred. Required Qualifications
- High school diploma or GED
- Strong communication and relationship-building skills
Desired Qualifications
- Knowledge of community resources preferred
