Community Health Worker

Job Summary
Community Health Worker supports outreach, resource coordination, and care navigation for community members; collaborates with the Case Manager and healthcare team to provide education, behavior modification, social support, and advocacy; bridges gaps between organizations and health services through grant-funded programs to improve access and health outcomes; responsibilities include connecting patients with community resources, increasing culturally relevant knowledge about health and wellbeing, and assisting with resource allocation and management.
Required Qualifications
- High school diploma or equivalent required
- Valid driver’s license required
- Knowledge of medical terminology preferred
- Community Health Worker certification preferred
- Experience working in a community-based setting preferred
- Experience identifying and coordinating community resources preferred
- Epic experience is a plus
- Spanish-speaking ability strongly preferred
