Job Summary
Community Health Worker (School-Based Health Center) helps connect students and families to healthcare, behavioral health, and community resources; conducts outreach and engagement with students and families; coordinates referrals and follow-up; supports health education, advocacy, and program quality improvement; ensures confidentiality per HIPAA/FERPA and adapts to social determinants affecting access to care; collaborates with school personnel, healthcare providers, and community partners to promote wellness and address barriers to health for children and adolescents.
Required Qualifications
- High school diploma or equivalent
- Experience working with children, adolescents, families, or community-based populations
- Strong interpersonal, communication, and relationship-building skills
- Proficiency with computers and electronic documentation systems
- Valid driver's license and reliable transportation, if required
Desired Qualifications
- Community Health Worker certification or willingness to obtain certification
- Associate's or bachelor's degree in public health, social work, human services, healthcare, education, or a related field
- Experience in school-based health, community health, care coordination, behavioral health, or social services
