Job Summary
The CHW-SBHC will establish trusting relationships with students, families, and school personnel to navigate health and social services at the School Based Health Center. They will assist with benefits enrollment, facilitate connections to primary care, behavioral health, and community resources, deliver COVID-19 education, coordinate referrals, and advocate for patients across school and medical homes. Responsibilities include maintaining patient care records in the EMR, supporting vaccination efforts, participating in health initiatives and wellness activities, and collaborating with school staff and behavioral health providers to promote comprehensive, coordinated care for students and families.
Required Qualifications
- Written and oral fluency; Bilingual English and Spanish preferred
- Commitment to and enthusiasm for working in a school setting
- Collaborative nature and ability to work with school and medical home staff
- Good communication skills (active listening, sincerity, tact)
- Ability to provide emotional support and motivation to students and families
- Complete all required trainings and background checks
