Job Summary

Conduct home visits and telephonic assessments to evaluate patient health, home environments, and barriers to care adherence. Develop and implement personalized care plans in collaboration with patients, families, and the clinical team while advocating for necessary healthcare resources and benefits. Coordinate primary and specialty care appointments, manage transportation logistics, and facilitate advanced care planning conversations. Document all assessments, observations, and progress notes accurately in multiple computer systems. Maintain client confidentiality and adhere to departmental workflows and training materials.

Required Qualifications

  • High School Diploma or equivalent experience
  • Associate's Degree or equivalent experience preferred
  • Community Health Worker Certificate preferred
  • Training or education in social work or benefits navigation preferred
  • Travel 50+% of the time in the assigned area
  • Must have a valid driver's license
  • Must have automobile insurance
  • Must have reliable transportation
  • 2-3 years of related experience in managed care, healthcare environment, lived experience or case management
  • 1+ year working with seniors, conducting home visits, and working remotely strongly preferred
  • Bilingual Spanish proficiency is preferred but not required
  • Basic technical skills for functional area
