Job Summary
Community Health Worker (CHW) to support outreach and care navigation for rural/medically underserved communities in Atlantic, Burlington, and Salem Counties as part of the Outreach Team. Responsibilities include engaging community members, promoting access to primary and specialty care, behavioral health, and social services; coordinating with Mobile Medical Unit activities; addressing barriers to care (transportation, insurance, food insecurity, housing instability, digital access); providing health education and facilitating telehealth; documenting encounters and outcomes in the EHR; and supporting quality improvement and HRSA program requirements. Bilingual Spanish skills preferred; flexible schedule with occasional evenings/weekends; HIPAA compliance and strong communication for diverse populations are expected.
Required Qualifications
- High school diploma or equivalent
- Valid driver’s license and reliable transportation
