Job Summary
Conduct in-home or telephonic visits to assess patients and home environments, identifying barriers to care and developing implementation plans in collaboration with patients, families, and the care team. Advocate for patients to access healthcare, benefits, and resources such as MediCal, drug subsidies, and transportation. Support advanced care planning, educate families on chronic disease management, and coordinate appointments and travel. Complete accurate documentation in multiple systems while maintaining patient confidentiality. Travel 50%+ of the time with a valid driver's license. Base salary $21.78 to $31.52 per hour.
Required Qualifications
- High School Diploma
- Valid driver's license
- Automobile insurance
- Reliable transportation
- 2-3 years of related experience in managed care, healthcare environment, lived experience or case management
- Tuberculosis screening proof or negative screening within the last year

- Training or education in social work or benefits navigation
- 1+ year working with seniors, conducting home visits, and working remotely
- Bilingual Spanish proficiency
