Job Summary
The RCCC Community Health Worker will provide outreach, care coordination, and access to social and medical resources for soon-to-release clients and their families over a 60-day period, maintaining a caseload of about 35 individuals. Responsibilities include conducting social needs screenings and health risk assessments, coordinating care with prison-based teams and HCAM stakeholders, documenting in electronic medical records, attending meetings at various facilities or client homes, using motivational interviewing to engage clients, delivering trauma-informed person-centered services, and providing ongoing post-release support (weekly for up to 2 months, then monthly for up to 12 months). Additional duties include educating uninsured clients about health insurance options (identifying an MCO and primary provider), referring clients to local programs in Maryland (Baltimore City/County or Anne Arundel County or other districts as needed), attending mandatory trainings, and complying with HIPAA and mandated reporting. Must be able to work flexible non-traditional hours.
Required Qualifications
- Minimum high school education or GED required
- Six (6) months’ experience working in a Community Health Worker (CHW) role or in another position with the provision of social services
- Active Maryland Community Health Worker Certification or the willingness to obtain certification within 12 months of hire
- Strong computer literacy (MS Word, Excel, Outlook, SharePoint 365)
- Excellent interpersonal and communication skills
- Reliable transportation
