Job Summary
Community Health Worker who connects PHA residents in Providence, RI to primary care, behavioral health, and community resources; conducts one-on-one assessments to address health and social needs, refer clients to services, and help set health-promoting goals. Will document progress in the case management database, maintain confidentiality, collaborate with the Wellness Team and community partners, and participate in ongoing program planning and network meetings. Bilingual Spanish/English required; prefers applicants living in public housing or Section 8 participants. This role involves working 35 hours per week in an office environment with capacity to lift up to 50 lb and perform sedentary and manual tasks; salary approx. $23.15/hour equivalent to $48,152/year, with full-time schedule and flexible hours as needed.
Required Qualifications
- High school diploma or GED
- CHWARI CORE Community Health Worker Certification is required
- Ability to speak Spanish and English fluently and to write English fluently
