Job Summary
Community Health Worker position focusing on culturally aligned, accessible care for Latino communities. Responsibilities include patient navigation and enrollment, outreach (virtual and in-person), coordinating care plans, scheduling appointments, linking patients to social determinants of health resources, educating patients on disease prevention, collaborating with a multidisciplinary care team, documenting interactions across systems, participating in community events, providing culturally/linguistically appropriate health education, assisting with federal/state program enrollment, supporting goal setting via motivational interviewing, conducting social needs assessments, and traveling to support community events and in-person patient appointments. Requires bilingual English/Spanish, CHW certification or equivalent experience, 1-3 years healthcare/navigation experience, and willingness to travel; fully remote work allowed within specified California locations.
Required Qualifications
- High school diploma or GED (minimum)
- Community Health Worker certification or 2000+ hours CHW work in past 3 years (and certification within 18 months)
- 1-3 years healthcare experience or healthcare navigation
- 2-5 years community work, advocacy, engagement, or organizing
- Familiarity with Google workspace (preferred)
- Past experience documenting in an EHR (preferred)
- Training in motivational interviewing (preferred)
- Fluency in English and Spanish (required)
