Community Health Worker, Patient Engagement

Job Summary
Build relationships with patients and caregivers through direct, in-person re-engagement in assigned geographic areas and conduct telephonic outreach to re-engage children in care. Provide hands-on support for program re-enrollment, onboarding, and mobile app setup while educating families on available community resources and social determinants of health. Partner with interdisciplinary teams and behavioral health providers to facilitate pharmacy access, coordinate primary care connections, and address barriers to service delivery. Perform community events and facility rounding to strengthen ongoing program participation, utilizing outbound calls as needed to support enrollment and follow-up metrics.
Required Qualifications
- A valid driver's license
- A bachelor's degree
- Ability to travel frequently
- 3+ years of experience in a healthcare setting (patient-facing hospital or facility role)
- Ability to navigate ambiguity, think critically, take initiative, and stay organized
- Ability to manage multiple tech platforms
- Experience working in a patient-facing hospital or facility role
- Experience working as part of an interdisciplinary team
Desired Qualifications
- National Community Health Worker Certification highly preferred
- Bilingual (Spanish) highly preferred
- Medical Interpreter Certification preferred
