Job Summary
Community Health Worker at Orchid Health focused on helping patients and families navigate community services and resources, engaging patients in their care plans, and supporting the Care Management Team to deliver whole-person care. Responsibilities include building trusting relationships with patients and families, initiating outreach around Social Determinants of Health (SDOH), following up on care plans and insurance coverage (OHP), coordinating services with the care team, connecting with local organizations, documenting contact in the EHR within 24 hours, and participating in team meetings and trainings. Strong emphasis on patient advocacy, community engagement, effective communication, and collaborating with healthcare providers to improve access to care and health outcomes. Required Qualifications
- Education: Minimum of high school diploma or GED; higher education preferred
- One (1) year relevant experience working with underserved populations with medical and social needs in a social service and/or community-based setting
- Basic computer skills, preference with EHR
- Good communication skills, such as listening well, and using language appropriately
- Ability and willingness to provide emotional support, encouragement and motivation to patients
