Community Health Worker or Housing Transition Coordinator

Job Summary
Serve as a liaison between healthcare and housing professionals to ensure smooth, person-centered transitions for adults moving from long-term care to community settings. Collaborate with case managers to arrange services aligned with consumer preferences, assist individuals in locating housing and obtaining necessary materials, and support access to entitled benefits. Maintain knowledge of service lines including adult day health, personal care, and hospice while addressing daily needs and ensuring client rights are upheld. Report any compliance issues immediately and adhere to all federal, state, and local regulations. This role requires frequent travel across Delaware, Licking, Union, Madison, Pickaway, and Fairfield counties to meet clients on the ground.
Required Qualifications
- High school diploma or GED
- Minimum of 1 year in social service or healthcare (STNA) delivery with adults, elderly and family populations
- Valid driver's license
- Automobile insurance
- Compliance with National Church Residences motor vehicle policy
- Knowledge of supportive services and community resources in assigned areas/counties
- Knowledge of housing services in the areas
- Monday through Friday availability (no weekends)
- In-person work in Franklin County
- Ability to travel to Delaware, Licking, Union, Madison, Pickaway and Fairfield Counties
Desired Qualifications
- Experience as an experienced Community Health Worker
- Experience as an STNA
- 2 years' experience in social service or healthcare delivery
- Knowledge of housing services in the areas preferred
