Job Summary
Conduct nonmedical home visits to complete surveys for patients and ensure they reside in safe environments. Travel to patient homes as determined by need and schedule, implementing patient care activities both initially and in six months per case. Maintain excellent time management and communication skills while managing deadlines. Requires own vehicle and valid driver's license to travel to Niagara Falls. Previous homecare experience helpful.
