Community Health Worker

Community Health Worker role supporting patients and families across clinic and community settings. Responsibilities include building relationships, developing care plans, assessing social determinants of health, connecting patients to health and social services, educating patients about eligibility and enrollment processes, scheduling and coordination assistance, and supporting continuity of care. Collaborates with clinical and community partners, documents in EHR, and conducts home-based or community-based care as needed. Local travel to patient homes and community sites is required. Fluency in Spanish preferred. THW certification required or willingness to obtain upon hire. Demonstrates independent work style, strong communication, and ability to navigate health and social service systems while advocating for patient needs.
