Job Summary
Conduct scheduled home visits with pregnant individuals, postpartum mothers, infants, and families to provide education on prenatal care, breastfeeding, nutrition, and safe sleep practices. Assess family needs regarding healthcare, housing, food security, and transportation barriers while connecting clients with healthcare providers, social services, and community resources. Assist clients in scheduling medical appointments and maintain accurate documentation of visits, assessments, and referrals in compliance with HIPAA standards. Collaborate with nurses, social workers, and community organizations to coordinate comprehensive care, participate in outreach events, and advocate for culturally responsive support. Requires a valid driver's license, reliable transportation, and willingness to travel; five-year grant funding supports this full-time role.
Required Qualifications
- High school diploma or GED
- Valid driver's license
- Reliable transportation
- Willingness to travel for home visits
Desired Qualifications
- Associate's degree in Health Sciences, Social Work, Public Health, or a related field
- Certification as a Community Health Worker
- Willingness to obtain certification as a Community Health Worker
- Experience working with pregnant individuals, mothers, infants, or families in community or healthcare settings
- Knowledge of maternal and child health
- Knowledge of community resources
- Knowledge of health education principles
- Bilingual abilities (English/Spanish or other languages)
- Experience with home visiting programs or case management
- Knowledge of trauma-informed care
- Knowledge of motivational interviewing
- Knowledge of family-centered care approaches
- CPR and First Aid certification
