Job Summary
Conduct in-depth assessments of patient needs, goals, and barriers to achieve good health outcomes. Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions. Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs. Assist patients in navigating the healthcare system and accessing appropriate services, including health insurance and social services. Provide ongoing follow-up and support to ensure continuity of care and successful achievement of health goals. Develop and implement health promotion programs and activities to address community health needs. Collect data and maintain accurate records of patient interactions and outcomes. Attend meetings and trainings related to community health promotion and education. Full-time, Monday through Friday, standard business hours with a hybrid work model.
Required Qualifications
- Bachelor's Degree Public Health, Social Work or Related Field of Study
- 2+ years of experience in community health outreach, health education, SDOH or related field
- Experience navigating patient social risks and barriers
Desired Qualifications
- Certified CHW with MA DPH
- Bilingual (Spanish)
- Bilingual Spanish skills
