Job Summary
Provide case management, outreach, health education and interpretation services. Assist in training students in the health professions and health care professionals regarding cultural issues and health care. Serve as an advocate for appropriate, quality health and social services. Work as part of the Patient Centered Health Care Home team to deliver patient-centered care. Essential duties include interpretation services, health education and counseling, assisting patients with intake and enrollment processes, connecting patients with entitlements and outside resources, documenting contacts, attending trainings, assisting with medication-refill coordination, front-desk duties, appointment scheduling and reminders, insurance verification, and clerical support. Extended duties cover safety observations and coordination of outreach activities; regular on-site attendance is required.
Required Qualifications
- High School Diploma or GED required
- Fluency in English and Marshallese required
- Ability to translate technical/medical instructions in English and Marshallese
- Basic computer skills
