Job Summary
Assist patients in accessing cancer screenings, follow-up care, and a full range of medical, behavioral health, and community services. Assess social drivers of health, advocate for patients facing barriers to care, and provide education and coaching to promote positive outcomes. Screen for program eligibility, complete care plans with the clinical team, and document all work in the Compass Rose module of EPIC. Maintain caseload size and meet monthly outreach and engagement productivity requirements while participating in required trainings and meetings. Support the development of culturally tailored health education materials and utilize web-based applications to address population management goals.
Required Qualifications
- High School Diploma/GED
- at least 1 year experience in a health service related field
- Bi-lingual- English/Spanish or Chinese, Arabic, Russian or Yiddish depending on targeted patient population
- Computer literate (Microsoft Office), telephone, fax machine, and photocopy machine skills
- able to effectively communicate with all levels of the organization
