Job Summary
Community Health Worker provides care coordination support with a preventative health focus for CalAIM clients, working with the Lead Care Manager. Responsibilities include maintaining regular client contact to ensure referral services are delivered, completing referrals and follow-up, conducting outreach to newly referred clients, providing culturally appropriate health/mental health education, performing home/hospital/facility visits, assisting with Medi-Cal waiver enrollment documentation, coordinating with Medicare/Medi-Cal programs, maintaining client confidentiality, and collaborating with the interdisciplinary team to support aging adults and adults with disabilities in the community. Requires HS Diploma and 2+ years of CHW experience; compensation range is $30-$32 per hour ($62,400-$66,560 annually). Also emphasizes scam awareness and official channels for application.
Required Qualifications
- HS Diploma or equivalent
- Community Health Worker certificate preferred
- At least 2 years experience as a Community Health Worker
- Familiarity with Medi-Cal, CalAIM, and related waiver programs
- Computer literacy and ability to maintain documentation
- Excellent written and verbal communication skills
- Ability to conduct home visits and coordinate with interdisciplinary team
