Community Health Worker - Inpatient Case Management

Community Health Worker in Inpatient Case Management at Sentara Virginia Beach General Hospital. Collaborates with the Continuum Case Manager and healthcare team to provide comprehensive and coordinated care, engages in interventions to improve self-management and quality of life for patients with chronic/complex illness, delivers education and behavior modification strategies, provides social support to patients and families, and serves as a patient advocate and healthcare navigator for resource allocation and management. Requires a high school diploma or equivalent, valid driver’s license for VA or NC, and prefers bilingual English/Spanish; knowledge of medical terminology and experience in community-based settings are preferred.
