Job Summary
Community Health Worker III supports patients across Alameda and Santa Clara Counties, engages in outreach, care coordination, case management, and linkage to resources at BACH and in the community. Responsibilities include housing and crisis assessments, SDOH assessments, intake for referrals to case management, monitoring and reporting staff productivity, facilitating training for CHW I-II, scheduling external/online training, preparing presentations, ensuring HIPAA compliance, building partnerships with community resources, promoting integrated care, and providing advocacy and follow-up across multiple sites (clinics, mobiles, schools, homeless encampments, warming centers, and patients’ homes) with flexible hours including some evenings and weekends.
Required Qualifications
- High school diploma or equivalent
- 2-4 years of experience in a community-based health care service capacity preferred
- AS/AA in health or related field preferred
- Bilingual preferred
- CEC Certification, if applicable
- CHW Certification - Recertification and/or retraining process, as required
