Job Summary
Provide advanced health education, outreach, and care coordination to individuals and families in the community. Responsibilities include conducting in-depth assessments, coaching for self-management of chronic conditions, collaborating with healthcare providers to implement care plans, navigating the healthcare system and accessing services (including health insurance and social services), providing ongoing follow-up, developing health promotion programs, and collecting/maintaining patient data. Ability to work in a hybrid setting at Chelsea Everett Ave site with a focus on promoting healthy behaviors, supporting care continuity, and addressing social determinants of health. Bilingual skills preferred and experience in community health outreach is valued.
Required Qualifications
- Education Bachelor's Degree Public Health required or Bachelor's Degree Social Work preferred or Bachelor's Degree Related Field of Study required
- Massachusetts CHW license preferred
- 2-3 years experience in community health outreach, health education, or related field
- Experience in advocacy for diverse populations
- Strong communication and interpersonal skills
- Basic computer skills including Microsoft Office and database management
Desired Qualifications
