Job Summary
Community Health Worker I responsible for client advocacy, care coordination, and health education to support patients in managing chronic conditions and participating in research. Performs home visits, assesses needs, facilitates referrals to health and social services, enrolls clients in health coverage programs (e.g., CHIP, Medicaid), recruits for research studies, and maintains confidential documentation. Works with multidisciplinary teams to connect clients with services, delivers health coaching (including smoking cessation), and supports self-management plans to improve health outcomes. Requires a high school diploma or equivalent and CHW certification; 0-1 years of related experience; duties include IRB protocol compliance, data entry, and travel within assigned area.
Required Qualifications
