• •High School Diploma or equivalent
• •Must be able to achieve and maintain Community Health Worker certification within 12 months of hire
• •Must possess/maintain a valid driver's license
• •Must possess/maintain current automobile insurance
• •Must possess a driving record that meets the guidelines and requirements of the organization
• •Must have reliable transportation
• •Must be able to utilize personal vehicle to transport self/others and to attend trainings and/or meetings
• •Must be able to work independently as well as collaboratively within a team setting
• •Must exhibit ability to work with targeted population
• •Must possess knowledge of and ability to relate to community resources
• •Must be able to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
• •Must be able to maintain strict confidentiality and handle highly confidential information with professionalism

• •Must be able to effectively match changing needs with appropriate services
• •Must be able to effectively refer to needed resources (primary care, behavioral health care, recovery servcies, SNAP, Social Security, transportation, employment, etc)
• •Must be able to effectively provide informal counseling
• •Must be able to effectively provide social support
• •Must be able to effectively provide advocacy
• •Must be able to effectively provide community education
• •Must be able to effectively provide outreach
• •Must be able to effectively improve health outcomes
