Job Summary
Community Health Worker (CHW) to engage and support patients in navigating Medicaid, healthcare access, and community resources. Responsibilities include engaging patients in primary and preventive care, supporting adherence to treatment plans, connecting to Medicaid providers and transportation or interpretation services, assisting with telehealth, delivering health education, helping develop care plans with the healthcare team, and promoting access to preventive services and resources. Level I pathway toward CHW certification by 12/31/2025; onsite at Heart of the City Health Center with occasional travel to other Cherry Health sites and occasional events outside typical hours. Requires strong communication, cultural liaison abilities, and independent work with diverse populations; proficient in Microsoft Office and NextGen EHR. Required Qualifications
- High School Diploma or Equivalent required
- Certification as a CHW from an accredited program or ability to attain certification by 12/31/2025
- Experience in healthcare or community settings preferred
