Job Summary

Community Health Worker to support HWCLI’s Social Care Network initiatives under the 1115 Medicaid Waiver. Responsibilities include conducting health-related social needs screenings, identifying and verifying eligibility, documenting in Unite Us and other systems, performing outreach via virtual, telephonic, or in-person means, making referrals to federal/state/local social care resources, developing care plans, and following up to ensure services are provided. The role involves resource navigation, assisting clients with access to health and social care services, ensuring timely screenings, maintaining documentation, and meeting monthly productivity expectations. Requires effective communication across diverse populations and the ability to coordinate with multiple partners to improve health equity. Hybrid work environment with travel to office and local partners as needed.

Required Qualifications

  • High school diploma or GED
  • 2-3 years of relevant work experience
  • Experience in the community health care setting OR health coach/patient navigator
  • Valid Driver's License preferred
  • Bilingual preferred
