Job Summary
Screen caregivers for social drivers of health and assist patients and families with benefits, entitlements, housing, and other needs impacting physical and emotional well-being. Advocate for patients facing barriers to care, including language, literacy, transportation, insurance, and appointment scheduling issues. Provide education on preventive health and ensure access to services indicated in individual education plans. Collaborate with school and community-based organization staff to coordinate services and increase enrollment in the School Based Health Center. Conduct outreach efforts to engage high-risk patients in case management while maintaining established caseload sizes and meeting monthly productivity requirements. Maintain electronic records, compile statistical data, and complete documentation within required time frames. Prepare for and participate in individual or team meetings and supervision as required. Utilize web-based applications and technologies to address population management goals.
Required Qualifications
- High School Diploma/GED
- at least 1 year experience in a health service related field
- Bi-lingual- English/Spanish or Chinese, Arabic, Russian or Yiddish depending on targeted patient population
- Computer literate (Microsoft Office), telephone, fax machine, and photocopy machine skills
Desired Qualifications
- Experience working with families with young children to address and health and social service needs
