Job Summary
Conduct intake interviews with patients, enrolling and referring them into local services while providing culturally appropriate health education. Assist patients with navigating community resources, completing applications, and connecting with transportation options. Perform home visits and phone follow-ups to support uninsured patients through eligibility determination and goal setting. Provide informal counseling on health behaviors and maintain trusting relationships with families. Work under direct supervision to ensure coordinated care from initial identification through closure.
Required Qualifications
- Successful completion of a Community Health Worker formal training program from an accredited college, or other educational institution
- Associate's/Technical Degree or equivalent combination of education/related experience
- High School Education/GED or equivalent
- Must be able to receive all required vaccinations as a condition of employment
- Must acknowledge and understand E-Verify Participation and Right to Work notices
- Must be available for day shift
- Must be available for full time employment
