Job Summary
Community Health Worker (CHW) to support the Health & Affordability Corps (H&AC) delivering connection to health coverage, public benefits, and essential social services. Serve as a trusted liaison to help New Yorkers access resources that promote health equity. Responsibilities include using a digital navigator tool to connect residents to resources, logging client encounters with billing codes, coordinating outreach and stakeholder relationships, surveying residents and leaders, planning health awareness activities (some evenings/weekends), collecting and entering data, documenting program efforts in databases, providing clerical support, delivering presentations/workshops, reporting program activities, managing resource distribution, assisting with applications for benefits, supervising volunteers, and other assigned duties. Qualifications emphasize a high school diploma with extensive relevant experience or a bachelor's degree with related experience; preferred bilingual capabilities and a commitment to addressing health inequities. Required Qualifications
- High school diploma or equivalent and six years of experience in community work or related duties; or
- Bachelor's degree and two years of related experience; or
- Equivalent combination of education and experience with at least one year of qualifying experience
