Job Summary
Community Health Worker (Field) to engage with members, conduct regular SDoH-focused assessments and visits (in homes, schools, clinics, shelters, and community centers), provide culturally appropriate health education and outreach, bridge gaps to health and human services, assist with navigating state/federal benefits and health insurance enrollment, advocate for addressing housing, health-related legal issues, and other social needs, support care plans with input from care teams, and participate in ongoing education and cross-functional collaboration.
Required Qualifications
- Credentialed CHW certificate or willingness to obtain within 1 year
- Bachelor’s degree in allied health preferred
- 3-5 years’ CHW experience in healthcare
- 1 year care coordination/care management
- Excellent communication skills
- MS Office
- Electronic Medical Record proficiency
- Managed care knowledge (Medicare/Medicaid)
- Knowledge of welfare reform, homelessness, community resources
- Bilingual or multilingual
Desired Qualifications
- Credentialed by the Board of Certification of Community Health Workers (CHW certificate) or willing to obtain with assistance of leadership team within one year of employment
- Bachelor’s degree in an allied health discipline (e.g., Health Education, Human Services, Public Health, Community Health, Social Work)
- 3 - 5 years' experience as a Community Health Worker in a similar position in a health care environment to include 1 year experience with care coordination and/or case/care management, advocacy, community health promotion, resource planning, and community-based visits
- Excellent written and verbal communication skills
- Working knowledge of Microsoft Office applications
- Ability to utilize an Electronic Medical Record
- Strong understanding of the managed care environment with focus on the Medicare/ Medicaid population
- Knowledge of welfare reform, homelessness, and community resources to address basic needs such as housing, food, transportation, clothing
- Knowledge of barriers encountered by low-income and culturally diverse families
- Demonstrated understanding of community services and navigating the healthcare system
- Bilingual or multi-lingual
- COVID policy awareness and compliance
