Job Summary
Community Health Worker stationed in Louisiana, traveling 60-75% to support Aetna members by conducting in-home visits, educating on care plans, and addressing social determinants of health. Acts as a bridge between members and healthcare providers, fostering culturally and linguistically appropriate care, assisting with care plan implementation, linking members to social services, and maintaining member records. Requires in-state travel, CHW certification within 12 months if not already certified, 1-2 years of clinical-support and 1 year of outreach experience, bilingual abilities a plus (Spanish or French). Full-time role with a pay range of $18.50-$31.72/hour, within CVS Health/Aetna benefits package; 40-hour work weeks with some after-hours flexibility and remote-work considerations limited to in-state travel needs. Must reside in Louisiana and be able to travel within specified regions; travel and use of electronic health records are essential components.
Required Qualifications
- Must reside in LA within Region 8, Region 1, Region 5, or Region 7
- Must successfully complete Certified Community Health Worker/Promotora certification within 12 months of start date (if not CHW certified at hire)
- 1-2 years’ experience providing clinical support services
- 1 year of community outreach experience and knowledge of LA resources
- 1 year experience working with clients with clinical needs (physical and/or behavioral health)
- Proficiency with MS Office Suite and EHR systems
- Flexibility to work outside core hours (Mon-Fri 8am-5pm) to meet member needs
- Access to reliable transportation
- Education: High school diploma or GED; CHW certification within 12 months
