Job Summary
Community Health Worker (Field) in Louisiana supporting CVS Health/Aetna members with chronic conditions. The role involves conducting in-home visits to reinforce care plans, educating providers on social determinants of health, linking members to community resources, assisting with care management, and maintaining electronic health records. Requires 60-75% travel, ability to work remotely within Louisiana, and coordination with interdisciplinary teams to reduce hospitalizations and improve health outcomes. Required Qualifications
- Must reside in LA and within the assigned region/parishes: Region 8 (Caldwell, East Carroll, Franklin, Jackson, Lincoln, Madison, Morehouse, Ouachita, Richland, Tensas, Union, and West Carroll); Region 1 (Orleans, Jefferson, St. Bernard, Plaquemines); or Region 5 (Beauregard, Allen, Jefferson Davis, Calcasieu, Cameron) Region 7 ( Caddo, Bossier, Webster, Claiborne, Bienville, Red River, DeSoto, Sabine, Red River, Natchitoches)
- If not CHW certified at time of hire, must successfully complete a Certified Community Health Worker /Promotora certification program as recognized by the State of Louisiana within 12 months of start date
- Ability to travel 60-75% of the time
- Must have access to reliable transportation
- 1-2 years’ experience providing clinical support services
- 1 years of community outreach experience and knowledge of LA community resources
- 1 years’ experience working with clients with clinical needs (physical and/or behavioral health)
- Demonstrated proficiency with a personal computer, keyboard navigation, and MS Office Suite applications (i.e., MS Teams, Outlook, Word, Excel, and other Electronic Health Record Systems)
- Flexibility to work outside of core business hours Monday-Friday, 8am-5pm, to meet member needs
- Education High school diploma or G.E.D. Certification/Licensure Community Health Worker/Promotora certification program as recognized by the State of Louisiana within 12 months of start date
