Job Summary

Required Qualifications

• •Reside in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties, or the surrounding service area
• •Willing and able to travel within a 90-mile radius up to 70% of the time
• •Possess reliable transportation and maintain the ability to travel throughout the assigned territory
• •1-3 years of relevant experience
• •Experience working with diverse urban and/or rural populations
• •Strong understanding of local community resources, support networks, and service organizations
• •Ability to communicate medical information to health care professionals and care coordinators over the telephone
• •Knowledge of formal and informal community agencies and resources
• •Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
• •Strong interpersonal and communication skills
• •Ability to work effectively with a wide range of constituencies in a diverse community
• •Ability to influence medical care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences
• •Ability to work collaboratively in a team
• •Effective time management skills
• •Sound judgment
• •Exceptional organizational and interpersonal skills

• •Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings
• •Dedicated workspace free of interruptions
• •Dependents must have separate care arrangements during work hours

Desired Qualifications

• •Experience working in community/outpatient setting
• •Community Health Worker Certification or equivalent training and experience
• •Mental Health First Aid
• •Trauma Informed Care
• •HIPAA
• •C.L.A.S. Standards
• •Motivational Interviewing
• •Specialized training in medical terminology and practices for designated health conditions
• •Experience using a computer including the use of Excel and Electronic Health Records
