Job Summary

Conduct community outreach and education to bridge members with the healthcare system, utilizing telephone communication and multi-system knowledge to improve health outcomes. Travel 70% of the time to Barry, Van Buren, Kalamazoo, and surrounding counties to support socially and medically complex patients in non-traditional settings, adjusting interventions based on cultural preferences. Manage multiple priorities and diverse constituencies while maintaining sound judgment in varying working conditions. Requires reliable transportation, a dedicated remote workspace, and 1-3 years of experience in community outreach or care coordination.

Required Qualifications

  • Reside in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties, or the surrounding service area
  • Willing and able to travel within a 90-mile radius up to 70% of the time
  • Possess reliable transportation and maintain the ability to travel throughout the assigned territory
  • 1-3 years of relevant experience
  • Experience working with diverse urban and/or rural populations
  • Strong understanding of local community resources, support networks, and service organizations
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone
  • Knowledge of formal and informal community agencies and resources
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
  • Strong interpersonal and communication skills
  • Ability to work effectively with a wide range of constituencies in a diverse community
  • Ability to influence medical care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences
  • Ability to work collaboratively in a team
  • Effective time management skills

  • Dependents must have separate care arrangements during work hours
  • Must reside in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties, or the surrounding service area

Desired Qualifications

  • Experience working in community/outpatient setting
  • Community Health Worker Certification or equivalent training and experience

  • HIPAA
  • C.L.A.S. Standards
  • Motivational Interviewing
  • Specialized training in medical terminology and practices for designated health conditions
  • Experience using a computer including the use of Excel and Electronic Health Records
