Job Summary
Community Health Worker at a federally qualified health center in Kalamazoo County assists patients and families in navigating community services, conducts intake and eligibility determinations, performs motivational interviewing and goal setting, enrolls or refers patients to programs, provides follow-up in person/clinic/through outreach, educates to remove barriers for preventive health screenings, connects patients with transportation resources, coordinates with providers for comprehensive, coordinated care, advocates between patients/families and community service agencies, documents outreach in EPIC and Azara within 24 hours of contact, and represents the organization at community events while managing an assigned caseload. Requires knowledge of medical terminology, effective communication across diverse populations, and proficiency with Microsoft Office; CHW certification or willingness to obtain during the first six months.
Required Qualifications
