Job Summary
Community Health Worker (CHW) responsible for helping patients and families navigate community services and resources, conducting intake interviews and enrollment/referrals, providing follow-up, educating on preventative health, connecting with transportation resources, and acting as a liaison between patients, families, and service agencies. Responsibilities include building trusting relationships, motivational interviewing, documenting outreach in EPIC/Azara within 24 hours, supporting care coordination with providers, managing an assigned caseload, attending community events and staff meetings, and supporting access to comprehensive, coordinated care for underserved populations in Kalamazoo County. Qualifications emphasize communication across diverse populations, familiarity with local resources, medical terminology knowledge, and CHW certification or commitment to obtain it; prior experience in case management or enrollment functions is preferred.
Required Qualifications
- One or more years of experience in case management, care coordination or enrollment functions
- CHW certification, or willing to obtain during first six months of employment
