Job Summary
Conduct outreach and engagement efforts to assist patients in self-advocacy and self-management across Niagara, Erie, Chautauqua, Cattaraugus, Allegany, Wyoming, Genesee, and Orleans counties. Accompany patients on clinical and supportive service appointments, assist with referrals, and link participants to care coordination and HIV medical care. Reach out to enrolled patients to discuss barriers and reengage those not virally suppressed through letters, home visits, or meeting patients at medical appointments. Assist the 50PLUS and AI MRT teams during specified interventions and health education sessions while tracking progress in work plans. This role supports Evergreen Health's mission to provide culturally competent care to underserved populations through collaborative, down-to-earth peer services.
Required Qualifications
- sensitivity to HIV/AIDS, addiction, and LGBTQIA+ issues
- awareness of and sensitivity to the issues and health disparities faced by underserved populations
Desired Qualifications
- high school diploma/GED
- personal or professional experience with HIV/Aging communities
- strong communication skills
- ability to do outreach at various times in a variety of locations
