Job Summary
Outreach and enroll eligible patients, building trusting relationships to create personalized goals that improve wellbeing. Provide tailored support in accessing health care, community connection, and social services while advocating for patient needs. Offer referrals for local social resources and communicate cultural concerns to the healthcare team. Requires a Community Health Worker certificate or ability to achieve within 18 months, plus at least one year in customer service or social service. Day shift rotation from 8:00 am to 4:30 pm with no weekend or holiday call obligation.
Required Qualifications
- At least 1 year of experience in customer service, social service, healthcare behavioral health or related field
- Community Health Worker certificate or ability to achieve within 18 months of employment or date of hire
