Job Summary
Mentors Community Health Workers and coordinates care for vulnerable community members through in-home visits, assessments, and referrals to social services. Responsibilities include overseeing care to improve health outcomes, providing health education about materials and general care, delivering community outreach, maintaining a daily data-entry routine, and advocating for members within the care team. The role emphasizes developing health management care plans, empowering members through problem solving and lifestyle changes, building positive relationships with diverse members, and delivering coaching on chronic health conditions and self-care. Essential duties align with ADA considerations and may involve other related tasks as required.
Required Qualifications
- High School Diploma or equivalent
- 1-3 years of relevant experience
- Community Health Worker certificate, highly preferred
