Job Summary
Supervise Community Health Workers across Public Health Nursing, Behavioral Health, Recovery, Maternal Child Health, and other specialty programs by providing direct leadership, mentoring, and support. Develop staffing schedules, coverage plans, and program priorities while conducting regular individual and team supervision meetings. Monitor caseloads, productivity, documentation, and service quality to ensure alignment with program goals, grant requirements, and departmental policies. Coordinate referrals between CHWs, healthcare providers, social service agencies, and community partners. Assist with recruitment, onboarding, training, and retention efforts, including annual performance evaluations and workforce development initiatives. Prepare monthly, quarterly, and annual reports on CHW activities, outcomes, and program impact. Represent the program at local, regional, state, and national meetings as assigned.
Required Qualifications
- Valid driver's license
- Meet driver's eligibility guidelines
- Home telephone/cell phone
- Competence in Microsoft Office programs (Word, Excel, Outlook, and RPMS)
- Two (2) years of college preferably in a Health-related area
- At least one (1) year of relevant experience as a supervisor
- Demonstrated leadership skills
- Knowledge of CHR philosophy and responsibility
- Ability to attend initial CHR training sessions in person or online periodically throughout employment
- Positive role model in the Winnebago Community
- Knowledge of the principles and practices of healthcare delivery system
- Ability to keep strictest confidentiality dealing with all patient records
- Compliance with the Federal Privacy Act
- Knowledge of the healthcare resources and services
- Knowledge of Medicare and Medicaid eligibility
- Knowledge of budgeting, principles and practices
- Ability to analyze healthcare problems and services
- Ability to keep accurate written reports on a daily, weekly, monthly schedule
- Above average social interaction skills
- Ability to read, write, and speak the English language fluently
- Ability to continually and effectively employ professional verbal and written communications skills
- Compliance with 45 CFR 1301, Subpart D, Section 1301.31, C and D regarding declaration of arrests and convictions of child abuse or violent felonies
- Compliance with PL 101-630 and PL 101-637 regarding criminal records check
- Ability to lift up to 10 lbs
- Ability to walk, stand, and sit for extended periods of time
- Ability to use hands to operate computer keyboard, phone, and various office equipment
- Ability to talk and hear
- Works with minimal to no supervision
